- If you think information is missing from your record or you want to request your hospital medical record, contact our Health Information Management (HIM) Department at (936) 569-4617. Hours are Monday through Friday 8:00 a.m. to 4:30 p.m. NOTE: There is a 36 hour delay for posting most information.
- If you think information is missing from your record or you want to request records from your physician, please contact the doctor's office directly.
- If you forgot your password, go to the log-on screen, enter your username and click the 'Forgot your Password' link. Enter the answers to your three security questions and reset your password.
